Person-Centered Thinking (PCT) CLASS DESCRIPTIONS

 

Support Development Associates, LLC, in partnership with The Council on Quality and Leadership and the Colorado Department of Health Care Policy and Financing, is pleased to offer a variety of person-centered thinking trainings around Colorado. These class descriptions are in draft format only and are still subject to approval by the Colorado Department of Health Care Policy and Financing.

2-Day Person-Centered Thinking Training
1-Day Introduction to Person-Centered Thinking

This 2-day, instructor-led training, focuses on changing perspectives from caring for people to providing balanced support between what is important to a person and what is important for a person. The two interactive days help participants change their thinking from a priority of fixing what is wrong with a person to supporting each person’s opportunities to share their gifts and live an everyday life. Eight specific tools for creating the structure of person centered thinking are taught across the two days. The first day’s focus is on exposure to the concepts; the second day provides experience in applied learning, intended to begin skill development.  There is no prerequisite for this course.  This course is appropriate for all people in direct support roles (staff, family, friends) as well as for administrators and policy makers and is offered at no cost to participants.

 

Participants will:

  • Learn to distinguish between Important To and Important For when discovering what represents balance in someone’s life.

  • Learn how to have conversations that get the most helpful information.

  • Learn the difference between supporting and fixing.

  • Learn how to develop a 1-page Person Centered Profile

This 1-day course is appropriate for all audiences and will consist of a brief overview of five essential person centered questions correlated to the CMS Final Rule.  Participants will be introduced to person centered discovery skills that will assist them in exploring the boundaries of choice in the context of determining what is important to people.  This course introduces 10 1-page profiles from across the age span and discusses a variety of issues for people using an array of supports and services.  There is no prerequisite for this course. 6 hours

 

Participants will:

  • Learn how Important To and Important For are used to represent balance in someone’s life.

  • Learn about the CMS Final Rule and how it impacts services and supports

  • Learn about the boundaries of choice in the context of discovering what is important to someone

  • Learn about the variety that exists in 1 Page Person Centered Profiles and how they are used in supporting people

1-Day Support Coordinators Partnering to Develop Outcomes

This 1-day course, custom-made for Support Coordinators, presents a clear process that can be used to move from person centered thinking to establishing goals and outcomes that are meaningful to the person and respectful of their preferences and desires while honoring CMS’ expectations of planning.  Completion of either Person Centered Thinking or Introduction to Person Centered Thinking is a prerequisite for this course. 6 hours

 

Participants will:

  • Learn how to use Important To and Important For to write plans that represent balance.

  • Learn about the CMS Final Rule and how it impacts services and supports.

  • Learn how to write outcomes that are in compliance with the Final Rule and honor what is important to the person.

  • Learn about the boundaries of choice and how to write plans that address risk respectfully.

  • Learn about using 1 Page Person Centered Profiles to fortify planning with people.

1-Day Families Developing One-Page Descriptions

This 1-day course, designed specifically for families and led by a family member, focuses on the family’s role as content experts (people in a person’s life who know the person well and care deeply about the person) in the development of person centered 1-page descriptions (or 1-page profiles).  The new CMS Final Rule requires person centered considerations in planning.  One-page profiles are a unique way to communicate plan details in a quick, concise format.  Participants will learn to use person centered skills and to organize the information we know and learn about the person into 1-page descriptions that represent a good balance and serve as a positive introduction for their loved one.  There is no prerequisite for this course, but, we suggest completion of either Person Centered Thinking or Introduction to Person Centered Thinking for those participants wanting to deepen their own knowledge base.  6 hours

 

Participants will:

  • Use person centered thinking skills to gain a deeper understanding of your family member… someone you love, care about, and support.

  • Learn the Core Concept:  Important To/Important For and Finding the Balance

  • Learn how to gather information using a variety of “Discovery Skills”

  • Learn how to organize and use that information to share with others by creating a One-Page Description and Communication Chart

 

* Due to the sensitive nature of sharing personal details in a one-page description, attendees of this course will be limited to individuals with IDD and their families. Providers may attend at the request of an attending family. 

2-Day Learning About People Using Personal Outcome Measures

This two-day, instructor-led training focuses on learning about people from the person and the people who know the person well. It is appropriate for all audiences. Participants will learn about quality of life and gathering information about what really matters. The instructor will conduct an interview with a person using the Personal Outcome Measures® and people who know the person and their supports well. Using information gathered, the instructor will guide a discussion about what’s really important and using that information to guide the person-centered plan. This Personal Outcome Measures® training will teach the learner the value of asking the right questions as a conversation. There is no prerequisite for this course. 6 hours

 

Participants will:

  • Learn about how Personal Outcome Measures® can be used to gather information from the person and people who know the person well.

  • Identify the person’s focus for person centered planning.

  • Develop a draft person centered plan using information gathered during the Personal Outcome Measures® conversation

1-Day Using Personal Outcome Measures to Learn From People Who Communicate Differently

This one-day, instructor-led training designed for Support Coordinators, Direct Support Professionals and other supporters will focus on learning about “listening” to people. Some people communicate differently. In order to effectively communicate, we have to use other techniques to gather information and learn about what is important to the person.  The session will use the Personal Outcome Measures® as a tool to gather information. This session will help the learner to improve “listening” skills by confirming discovery, practicing listening skills and communicating more effectively with people who communicate in different ways. There is no prerequisite for this course. 6 hours

 

Participants will:

  • Improve listening skills, confirm discovery, practice skills and communicate more effectively with people who communicate differently.

  • Gain experience in understanding non-verbal communication.

  • Demonstrate how to confirm what they have learned

  • Identify strategies for gathering information from a person who communicates differently.
