Are you looking for a great human services position and enjoy the administrative side of the field? Developmental Pathways has a great opportunity for an Active/ Enrollment Case Manager. You will be responsible for verifying eligibility and developing, monitoring, and assuring implementation of Service Plans (SP). Our Case Managers get to enroll all ages on to their caseloads. They provide ongoing case management, facilitate Interdisciplinary Teams (IDTs) while also assuring any services and supports are coordinated.
We have two office locations in the Denver, CO Metro area – one in Aurora, CO ( Exit 5 off of I-225) and one in Englewood, CO (Exit 195 off of I-25). The office location for this opening will be dependent upon the team needs at each location.
Our Case Management team has implemented a mobile working environment to provide the team with more flexibility in their roles. We’re committed to a great work-life balance here, so our offices are open Monday – Thursday and are closed on Fridays! This is a full-time 36-hour/week position working Mondays – Thursdays.

$18.00 - $21.00 / hourly or $19.05 - $22.05 / hour for Bilingual Case Managers

Ensure the accuracy, timely entry and distribution of written Service Plans
Complete targeted case management activities
Coordinate services and supports for individuals in services
Implement and lead Interdisciplinary Team Meetings as appropriate
Monitor and evaluate effectiveness of services provided by the Interdisciplinary Team and assure outcomes are met
Advocate for service needs of individuals
Coordinate support in emergency and crisis situations
Serve as liaison for individuals in services’ issues between Case Management and Pathways staff
Provide support and information for families of individuals in services as appropriate
Establish positive working relationships with legal entities, mental health professionals and other involved parties
Must haves to be our Case Manager!
Bachelor’s degree in human services related field, required
OR 5+ years of professional experience working specifically with the long-term services and supports (LTSS) population
You will be required to provide a copy of your transcript(s) (official or unofficial) or a copy of your diploma(s)
One (1) year of experience in human services or a related field
Working knowledge of the developmental disabilities system, rules and regulations preferred
Ability to comfortably learn and navigate technology (online databases, software systems and programs)
Excellent oral and written communication skills
Desire and dedication to provide the best customer service
Strong organizational and negotiation skills
Personal vehicle in good operating condition for use during work
Valid Colorado motor vehicle license/proof of adequate motor vehicle insurance
Meet our driving requirements - no suspensions and no more than two (2) tickets in the past three (3) years
